The phrases

sarvakarmāṇy api sadā kurvāṇaḥ (सर्वकर्माण्यपि सदा कुर्वाणः) is the man still in action. Even doing all actions, always. The participle is nominative, the subject.

madvyapāśrayaḥ (मद्व्यपाश्रयः) describes that same man. His refuge is me.

matprasādād avāpnoti śāśvataṃ padam avyayam (मत्प्रसादादवाप्नोति शाश्वतं पदमव्ययम्) is what he reaches, and from where. By my grace, the perpetual place that does not perish.

Sanskrit in this line

The participle kurvāṇaḥ (कुर्वाणः) is middle: he is doing, and the doing concerns himself. Before the voiced m (म्) of mat (मत्), aḥ (अः) becomes o (ओ), so kurvāṇo madvyapāśrayaḥ (कुर्वाणो मद्व्यपाश्रयः). Then t (त्) of mat (मत्) before the vowel of avāpnoti (अवाप्नोति) is voiced to d (द्): प्रसादात् + अवाप्नोति prasādāt + avāpnoti is prasādād avāpnoti (प्रसादादवाप्नोति).

Say it back

Cover the English. Say what he is still doing, and what place he attains. If the actions have to stop before the place is reached, stay here.
